THE ISSUES IN THE CASE — UNINSURED MOTORISTS COVERAGE
The parties in this case are the plaintiff, [Plaintiff], and the defendant,
[Defendant]. This case arises out of an auto accident between [Plaintiff] and [Tortfeasor]
that occurred on [Date] at [Location]. [Plaintiff] seek s to recover under an insurance
policy that the [Defendant] issued in which it agreed to pay [Plaintiff] for injuries from
auto accidents where another driver is at fault.

The issues in this case for you to determine are:
1. Whether [the Tortfeasor] is legally responsible for the accident, and, if so,
2. What injury, if any, did [Plaintiff] sustain as a dir ect result of the accident
and what damages [Plaintiff] should recover as compensation for the
injury.
