10JDCR 3.5
Proof of service
A proof of service will contain:
(a) The exact name of the document being served;
(b) The manner of service, e.g., mailed or hand-delivered;
(c) If service is made by mail, the name and mailing address of the person served; if hand-delivered, the name and location of the person served;
(d) The date of service; and
(e) The signature and printed name of the person making the service, and if hand-delivered, the relationship between the party having the pleading or paper served and the person serving the document.
