Monitoring Care

Members of the hospice IDG shall monitor the beneficiary’s condition and initiate changes

in the plan of care as needed. The beneficiary’s attending physician also participates in

this process. The IDG shall complete the review and resulting updates to the plan of care

every 15 calendar days to ensure that the beneficiary’s needs are met and shall document

each review in the beneficiary’s health record.
