HOSPICE CARE

PROVIDED

BY SNF/NF OR ICF/IID

FACILITIES

In addition to meeting conditions of participation that are found in

this hospice policy manual, a hospice that provides hospice care to

residents of Skilled Nursing Facility/Nursing Facility (SNF/NF

Intermediate Care Facilities for Individuals with Intellectual

Disabilities (ICF/IID) must be able to abide by standards provided in

42 CFR 418.112, which was adopted and incorporated by

reference by the Department.
