97-23-93] in _____________ [list court and details of previous conviction] on __________ [list
date of previous conviction] and of _________________ [specify violation of § 97-23-93] in
_____________ [list court and details of previous conviction] on __________ [list date of
previous conviction]; and
4. The two prior convictions for _________________ [specify violations of § 97-23-93] occurred
