Fraudulent Use of Social Security Number or Other Identifying Information - False Information about Identity

number or other identifying information.
If you find beyond a reasonable doubt from the evidence in this case that:
1. On or about [date of alleged crime], in __________ County;
2A. [Name of defendant] [intentionally/willfully/knowingly] and unlawfully [furnished/gave]
false information about [his/her] identity, intending to deceive [name of person] as to [name of
defendant]'s true identity;
OR
2B. [Name of defendant] [intentionally/willfully/knowingly] and unlawfully [furnished/gave]
false information as to [name of person]'s identity, intending to deceive [name of person] as to
[name of person]'s true identity,
then you shall find [name of defendant] guilty as charged.
If the State did not prove any one of the above listed elements beyond a reasonable doubt,
then you shall find [name of defendant] not guilty of count ___.
