Aggravated Domestic Violence - Intentionally Causing Injury to a Person with a Deadly Weapon

If you find beyond a reasonable doubt from the evidence in this case that:
1. On or about [date of alleged crime], in ____________ County;
2. [Name of defendant] [intentionally/knowingly/purposely] and unlawfully [injured/caused

to___________________________________ [describe defendant’s alleged actions]; and

as a weapon under the existing circumstances, is reasonably capable or likely to produce death or
serious bodily harm to another person.
