Diagnosis Codes

The diagnosis code is required and must be entered on the claim form and the Hospice Election Statement exactly as it appears in the current International Classification of Diseases (ICD) reference book. Note that the appropriate code(s) must be coded to the highest level of specificity and used as applicable to the patient’s diagnosis(es). Claims may be denied if the diagnosis(es) is not coded to the highest level of specificity. The ICD Tabular List should be used as a guide in selecting the specificity of the appropriate diagnosis code. Diagnosis codes are not included in this section. The current ICD reference book should be used as a guide in the selection of the appropriate diagnosis code.
