Only the elected hospice provider identified on the Hospice Transaction (DHS-2868) and in the MHCP member’s file may bill MHCP for hospice care. Refer to the current Uniform Billing Editor (UB) guide (UB-04 Data Specifications Manual: Order by contacting NUBC) and the Centers for Medicare & Medicaid Services Medicare Claims Processing Manual for coding descriptions and requirements.
