Home and Community Based Waiver Beneficiaries (Children’s Waiver, Habilitation Supports Waiver)

If waiver services are not related to the terminal illness, the hospice should send the

MDHHS waiver program coordinator an explanation of the situation when enrolling the

beneficiary. The hospice agency should contact the waiver case manager or supports

coordinator at the PIHP/CMHSP to coordinate care and to develop a combined POC.
