Nursing Facility Setting
Non-emergency transportation related to the terminal illness is the responsibility of the
hospice agency.
Routine, non-emergency transportation not related to the terminal illness must be
provided by the NF as part of their per diem.
Non-emergency ambulance transportation not related to the terminal illness requires a
signed physician's order and may be billed directly to Medicaid by the ambulance
provider. If the NF does not have a physician's order, neither the NF nor the ambulance
provider can bill Medicaid, the beneficiary, the beneficiary's family, or use the offset to
the patient pay amount. Arrangement for payment is between the NF and the
ambulance provider.
Emergency transportation related to the terminal illness is the responsibility of the

directly to Medicaid by the ambulance provider.
