Beneficiary No Longer Meets Admission Criteria
A beneficiary may have a change in condition and no longer qualify for hospice services. If the
beneficiary is discharged for this reason, the hospice must complete the discharge in CHAMPS, provide
the date of the discharge, and indicate the reason in the Remarks section of the webpage for the
Terminations generated by the hospice are subject to the appeal procedures, as
required by licensure requirements.
