Submission of work/exposure history
Plaintiff shall submit a statement of the injured
person’s work history, including dates of service and identification of each work site at
which exposure to asbestos is alleged. If exposure is alleged at any place other than a work
site, plaintiff shall submit a statement identifying each site of exposure with dates of
exposure.
