IN RE: CIVIL COVER SHEET
ORDER
I. Introduction
Under Administrative Order 2019-17, use of a cover sheet for all district
and circuit court civil case filings became mandatory on January 1, 2021.
II. Purpose
The purpose of the civil cover sheet is to ensure proper identification of
civil case types and support uniform collection of data across the
Commonwealth. Data standards will improve court statistics and allow the
Kentucky Court of Justice to better manage current programs and future
initiatives.
III. Implementation
Cases that are filed conventionally (i.e., in paper format) must be
accompanied by a civil cover sheet. For cases that are filed electronically, the
Kentucky Court of Justice eFiling system will automatically collect the
necessary information from the filing party. Accordingly, a separate civil cover
sheet is not required.
