Order of Probation

(a) Purpose. The Supreme Court adopts the order form created by the Office of Judicial Administration and the Department of Corrections under K.S.A. 21-6607(d) for use as an order of probation when a district court places an offender on probation with court services.

(b) Mandatory Use. A court services officer must use the order of probation adopted under this rule to document conditions of supervision and any other orders described in subsection (d).

(c) Permissive Use. Nothing in this rule prohibits a probation officer from using the order of probation adopted under this rule when a district court places an offender on probation with an entity other than court services.

(d) Required Information. When a district court places on offender on probation with court services, the court services officer must complete the following information on the order of probation:

(1) check the box for any listed condition that the court orders;

(2) document any modification to a listed condition or special condition that the court orders;

(3) check the box for any financial obligation that the court orders and document the amount; and

(4) document any additional information.

(e) Signatures. The offender, court services officer, and judge must sign the order of probation.

(f) Filing. The court services officer must file the order of probation with the district court.
