Hospice Services for Hoosier Care Connect Members

Members enrolled in Hoosier Care Connect are eligible to receive all hospice levels of service available

under the hospice benefit: routine home care, continuous home care, inpatient respite care and general

inpatient care.

Hoosier Care Connect hospice benefits mirror those of Traditional Medicaid. Covered benefits include the

hospice core services covered under the Medicaid hospice per diem. Services included in the per diem that

are not considered core services are noncovered for these members.
