Ind. IHCP Hospice Services Provider Reference Module, Section 2, Medicare-Certified Hospice Providers
Medicare-Certified Hospice Providers
Providers that already meet standards for Medicare hospice participation and are licensed to provide
hospice care in Indiana can enroll directly as IHCP hospice providers by completing an IHCP Hospital and
Facility Provider Enrollment and Profile Maintenance Packet or by submitting an application through the
IHCP Portal. In addition, the IDOH must also send a copy of the provider’s C&T directly to the IHCP
Provider Enrollment Unit. Verification of a current Indiana state hospice license or approval is also
required with the IHCP application.
Requirements of the Affordable Care Act (see the Affordable Care Act Risk Category Requirements section)
must be satisfactorily completed before an enrollment is approved.
A hospice provider entitled to reimbursement by the IHCP is defined as a pub lic or private organization,
or subdivision of either, primarily engaged in providing care to terminally ill individuals and their families.
The organization is certified under Medicare hospice conditions of participation after completing Indiana
state hospice licensure requirements and has a valid IHCP Provider Agreement indicating intent to provide
hospice services.
