A pportionment of Respon sibility--Complaint and Claims for Contribution Tried
Concurrently (Same Issues)
If you find that [a ny of the defendants][the defendant] [are] [is] lega lly responsible f or
proximately causing pla intiff's [injuries] [damages], then y ou must appor tion damages by
determining the relative degree of legal responsibility of each [per son] [and] [e ntity] named or
described on the Verdict Form.
In making that determination, you should consider the deg ree to which the [condition of
the produc t] [a nd] [per son's] [a nd] [e ntity's] conduct prox imately caused pla intiff's [injuries]
[damages].
In your ver dict form, you will state the percentage of legal responsibility of each of the
[persons] [and] [entities] named on the ver dict form. The total of these percentages must add up
to 100%.
