Payment Adjustments

Policy and procedures regarding payment adjustments are provided in Chapter 100.

Adjustments may be initiated only for a service for which payment has been made

by the Department and reported on the Remittance Advice. It cannot be used to

correct a rejected service or a suspended claim. Hospices are to use the HFS 2249

adjustment form for services previously paid. Completed adjustment forms should be

mailed to the following address at the Department for processing:
