Plan of Care

A plan of care must be established and reviewed at least monthly and include all covered

services and supplies. Only services and items consistent with the plan of care are covered.

The initial plan of care must be written by a member of the basic interdisciplinary group who

assess the participant’s needs, after consulting with at least one other group member. One of

the caregivers developing the plan of care must be a nurse or physician. Hospice services are

not covered before the day the plan of care is established. The other two members of the

basic interdisciplinary group must review the initial plan of care and provide input within two

calendar days following the day of assessment.
