Idaho Medicaid Provider Handbook, Home Health and Hospice Services § 3.6.1
Orders
All home health services must be ordered by a physician, nurse practitioner, clinical nurse
specialist or physician assistant as part of a home health Plan of Care. Home health orders
must include the provider’s National Provider Identifier (NPI), the services or items to be
provided, the frequency, and, where applicable, the expected duration of time for which
services will be needed. Services and durable medical equipment and supplies (DME and DMS)
can have additional requirements in their specific provider handbook chapters. Services
required for extended periods must be reordered every 60 days except DME and DMS, which
must be reordered annually.
