Advanced Care Planning
Advanced care planning, or Advanced directives, is a covered service. Advance directives are
documents appointing an agent and/or documenting the participant’s decisions regarding
their medical treatment should they lack the capability to communicate their wishes in the
future. Planning may include Health Care Proxy, Durable Power of Attorney for Health Care,
Living Will, or Medical Orders for Life-Sustaining Treatment. Home health and hospice service
providers must explain to each participant their right to make decisions regarding their
medical care. It is recommended the Idaho Physician Orders for Scope of Treatment (POST)
Form be completed and placed at the care location, so the participant’s end-of-life wishes are
honored. The POST Form and other resources are available from the Idaho Secretary of
State’s Office’s health care directive registry webpage.
