Outpatient Hospital and Ambulatory Surgical Center Procedures Requiring Prior Approval

Please refer to the Part II Policies and Procedures for Physician Services Appendix E, O and L (Radiology Codes)

for a list of procedure codes that must be pre-certified and/or prior approved before services are rendered in the

Outpatient Hospital or Ambulatory Surgical Center setting.

Note that Prior Approval for some procedures, including but not limited to Tonsillectomies and/or

Adenoidectomies, Hysterectomies, and Cataract procedures, can be completed by telephone, written submission,

or web portal submission. Other procedures are limited to submission in writing or via web portal. For further

free).
