Requirements of Coverage

To be covered, a certification that the individual is terminally ill must have been completed as set

forth in Section 704 and hospice services must be reasonable and necessary for palliative care

management of the terminal illness and related conditions. The individual must elect hospice care in

accordance with Section 702. A plan of care must be established before services are provided. To be

covered, services must be rendered consistent with the plan of care. Only individuals through the age

of twenty (20) may receive curative care management concurrent with palliative care.
