General Criteria

For information on general documentation requirements, please refer to Florida Medicaid’s

recordkeeping and documentation policy.

Specific Criteria

Providers must maintain a fully executed, age-appropriate, election statement in the

recipient’s file in accordance with the specifications in 42 CFR 418.24.
