Stricken Evidence
The (testimony) (or) (evidence) offered by (name of witness) as to (description of fact
in issue) has been rejected by this Court and is stricken. You are not to consider for any
purpose any offer of evidence that is rejected or stricken. Such (testimony) (or) (evidence)
is to be treated as if you had never (heard) (or) (seen) it.
