TheproviderislicensedbytheColoradoDepartmentofRegulatoryAgenciesorthelicensingagencyofthestateinwhichtheproviderpractices.
Theservicesarewithinthescopeoftheprovider'spractice.
Theproviderisknowledgeableaboutgender-diverseidentitiesandexpressionsandtheassessmentandtreatmentofgenderdysphoria.
HealthFirstColoradoprovidesgender-affirmingcareservicesbenefitstoeligiblemembers.ProvidersshouldrefertotheCodeofColoradoRegulations,ProgramRules(10CCR2505-108.735),forspecificinformationwhenprovidingthesebenefits.
