Overview
HomeandCommunity-BasedServices(HCBS)arecertainHealthFirstColorado(Colorado'sMedicaidprogram)benefitsavailabletoMembersintheirhomesandcommunitiesasanalternativetoinstitutionalcare.HCBSprogramsforpersonswithdevelopmentaldisabilitiesincludetheChildren'sHabilitationResidentialProgram(HCBS-CHRP).
WaiverprogramsprovideadditionalHealthFirstColoradobenefitstospecificpopulationswhomeetspecialeligibilitycriteria.
LevelofcaredeterminationsaremadeannuallybytheCaseManagementAgenciesservingeachofthe20DesignatedServiceAreasstatewide.Membersmustmeetfinancial,medical,andprogramcriteriatoaccessservicesunderawaiver.Theapplicantmustbeatriskofplacementinanursingfacility,hospital,orICF/IID(intermediatecarefacilityforIndividualswithIntellectualDisabilities).Tousewaiverbenefits,Membersmustbewillingtoreceiveservicesintheirhomesorcommunities.AMemberwhoreceivesservicesthroughawaiverisalsoeligibleforallbasicHealthFirstColoradocoveredservicesexceptnursingfacilityandlong-termhospitalcare.
Applicantsmayapplyformorethanone(1)waiverbutmayonlyreceiveservicesthroughone(1)waiveratatime.
