TheCommunityFirstChoice(CFC)benefitprovidesavarietyofservicestoindividualsacrosstheagespectrum,asanalternativetonursingfacility,andinpatienthospitalstoqualifiedMembers.MembersmeetingprogrameligibilityrequirementsarecertifiedbytheCaseManagementAgency(CMA)aseligiblefortheseservices.BelowisadescriptionofservicesavailabletoeligibleMembersonCFC.WhetheramemberreceivesanyoftheservicesbelowdependsontheMember’suniquesituationandserviceplan.

ElectronicMonitoring(MedicationReminders)-Medicationremindersaredevices,controls,orapplianceswhichenableanindividualathighriskofinstitutionalizationtoincreasetheirabilitiestoperformactivitiesofdailyliving,suchasmedicationadministration.Medicationremindersshallincludedevicesoritemsthatremindorsignalthemembertotakeprescribedmedications.Medicationremindersmayincludeotherdevicesnecessaryfortheproperfunctioningofsuchitemsandmayalsoincludedurableandnon-durablemedicalequipmentnotavailableasaStateplanbenefit.

Homemaker-Servicesconsistingofgeneralhouseholdactivitiestomaintainahealthyandsafehomeenvironmentforthememberandprovidedbyatrainedhomemaker.Providedwhentheindividualregularlyresponsiblefortheseactivitiesistemporarilyabsentorunabletomanagethehomeandcareforhimorherselforothersinthehome.

PersonalCare-Servicesprovidedtoaneligiblemembertomeetthemember’sphysical,maintenance,andsupportiveneedsthroughhands-onassistance,supervision,and/orcueing.Theseservicesdonotrequireanurse’ssupervisionorphysician’sorder.Servicesareincidentaltothecareprovidedorareessentialtothehealthandwelfareoftheindividual,ratherthantheindividual'sfamily.

PersonalEmergencyResponseSystem(PERS)–Ongoingremotemonitoringthroughadevicedesignedtosignaltrainedalarmmonitoringpersonnelinanemergencysituation.Includestheinstallation,purchase,orrentalofelectronicmonitoringdevice.Deviceisrequiredbecauseofamember’sillness,impairment,ordisabilityandisessentialtopreventinstitutionalizationofthemember.

Consumer-DirectedAttendantSupportServices(CDASS)-CDASSisaservicedeliveryoptionthatoffersCFCMemberstheopportunitytodirectpersonalcare,homemaker,andhealthmaintenancetasks.Membersmayalsodesignateanauthorizedrepresentativetodirecttheseactivitiesontheirbehalf.

HomeDeliveredMeals–Deliveryofmealstomemberswhohavedietaryrestrictionsorspecificnutritionalneeds,areunabletopreparetheirownmeals,andhavelimitedornooutsideassistance.Atthemember’srequest,thisservicemayalsoincludenutritionalcounselingandmealplanning.MustdemonstrateneedfortheserviceinServicePlan:lacksoutsideassistance,services,orresourcesthroughwhichtheycanaccessmeals,isunabletopreparemeals,andthemember’sinabilitytoaccessorpreparemealsdemonstratesarisktothemember’health,safety,orabilitytoliveinthehomeorcommunity.HomeDeliveredMealsarelimitedtotwo(2)mealsperdayupto14mealsperweek,upto365days.HomeDeliveredMealsmaybeauthorizedpast365daysonacasebycasebasisifthereisademonstratedneed.

RemoteSupports–provisionofsupportbystaffataHIPAAcompliantMonitoringBasewhoengagewiththeMemberthroughlivetwo-waycommunicationtoprovidepromptsandrespondtotheMember’shealth,safety,andotherneedsidentifiedintheMember’ssupportplan.ThesesupportscanhelpaMemberlivemoreindependentlyintheirhomeandcommunity.RemoteSupportsTechnologyisincludedtopurchaseandinstalldevicesthatsupportthedeliveryofRemoteSupports.

TransitionSetup-Coordinationandcoverageofone-time,non-recurringexpensestoestablishabasichouseholdupontransitionfromaninstitutionalsettingorRegionalCentertoacommunitylivingarrangementnotoperatedbythestate.Note:ThisisnotavailablefortransitiontoProvider-ownedsettings(e.g.,AlternativeCareFacility[ACF],HostHome,GroupHome,orSupportedLivingProgram[SLP].Thisisnotavailableinacommunity-to-communitytransition).

HealthMaintenanceActivities–includesroutineandrepetitivehealth-relatedtaskswhicharenecessaryforhealthandnormalbodilyfunctioningthatapersonwithadisabilityisunabletophysicallycarryout.
