Eligible Hospice Providers

Hospice providers may include the following:

• ‹‹Hospices››

• Hospitals

• Skilled nursing facilities

• Intermediate care facilities

• Home health agencies

• Any licensed health provider who has been certified by the federal Medicare program

to provide hospice care and is enrolled as a Medi-Cal hospice provider

‹‹All Medi-Cal covered hospice services must be rendered in accordance with all Medi-Cal

policy as well as any applicable Centers for Medicare & Medicaid Services (CMS)

requirements.››
