Failure to Register as a Sex Offender or Child Kidnapper - Second Degree

, the defendant in this case, has been charged with the crime of failure to register as a sex offender or child kidnapper in the second degree.

To prove that the defendant committed this crime, the state must prove beyond a reasonable doubt each of the following elements:

(1) the defendant is a [sex offender] [child kidnapper];

(2) the defendant was aware of the circumstances that triggered his duty to [file a written notice of change of address] [file a[n] [annual] [quarterly] written verification] [supply all of the information required to be submitted to [register as a [sex offender] [child kidnapper]] [file a written notice of change of address] [file a[n] [annual] [quarterly] written verification]];

(3) the defendant failed to [file a written notice of change of address] [file a[n] [annual] [quarterly] written verification] [supply all of the information required to be submitted to [register as a [sex offender] [child kidnapper]] [file a written notice of change of address] [file a[n] [annual] [quarterly] written verification]]; and

(4) the defendant did so knowingly.
